Students must print forms #3, #8, #9, #10, #11, #12.
Provide the following with your submission:

• A copy of your Driver’s License or State ID

• Copies of any transcripts and certificates you have previously obtained

DOWNLOAD the Adobe Scan app to scan your documents properly.

⚠️ Photocopies or unclear scans will NOT be accepted.

Documents #2, #4, #5, #6, and #7 are for you to READ and KEEP for your personal records.
